Residents who have not registered may still register by completing a consent form. Forms are available in the front lobby. When you come to complete the consent form you need to bring your Medicare and insurance cards.
With this vaccination clinic we will have approximately 130 IL residents vaccinated. If you have been vaccinated or are scheduled to be vaccinated at another location please contact Beth Cataldi at (215) 302-3741 for us to register your vaccination information.
Please help us: “Resist the Urge and Help Fight the Surge!” There is to be no gathering within apartments or common areas.
